I'm not an expert. Tom, Yavonne, Wellington would be better qualified to answer that. And many others here. But from being here and reading it could be the lights are the wrong brand or type. I also agree with the suggestion of soaking him for 30 minutes in a little bit less than chin-deep warm water. I would suggest repeating the 30-minute soaks every 1-2 days for a while until he gets better. But I agree with determining more information with the questions about diet, heat, etc. above.What are the enclosures temps.
All over temp, basking temp, night temp?
Humidity?
What kind of heat and lighting are you using?
What kind of temp and humidity gauge?
Are you soaking him? If not do so ASAP in warm water for 30 minutes.
Size of enclosure?
Diet?

Your tortoise is slowly dying because its needs are not being met. I'm not saying that to be hurtful or mean. I'm saying that so that you understand what is wrong and how to fix it and save your tortoise.Thank you for the source, looking into that asap.
- Its not a 50 Watt LED light btw its a ceramic heat emitter, the light source is a LED light.
The Substrate is wood mixed with hay (picture attached for reference).
Unfortunately its very hard to find a good vets that handles reptiles/tortoises near my cause I live in Denmark and our vets only deals with dogs and cats, its very uncommon to own reptiles/tortoises here..

You need a flood bulb of some sort. Arcadia makes them, if they don't sell them in your hardware stores anymore there. He needs a basking area to warm up ASAP. Next is and Arcadia ProT5 kit for UVB, and a calcium supplement with D3. Then, if he's still eating, get him the right foods. Generally no, those bulb are not recommended. In your case, I think it might be an okay way to go in the short term. It will instantly get him the warmth and UV that he needs, and since he looks close to full grown, I don't think it will hurt him as far as causing pyramiding.
