Hermans tort

Ive had a hermans tortoice for 2 and half years ,always been active healthy etc .past month or so hes been less active and eating less
Then past week he wont come out just burrows in corner ,if i get him out and put him under heat to warm up he just goes back into other corner hasnt eaten for at least a week.very worried.

Would you mind answering these questions? It’ll help a bunch for members to advise

What are your temperatures like? Ie basking spot(directly under the bulb) overall day and night temps?
What kind of lighting&heating are you using specifically? Packaging pics are good if you have any!
What kind of indoor uv bulb are you using?
How’s your humidity reading? What do you use for substrate?
