I agree. The only thing I have seen where a tortoise will actually do that "scream" - is a stone they cannot pass. Go to a vet and have an x-ray done to see if there is a large stone it cannot pass.I have heard about Tortoises screaming like that when they have a bladder stone they are trying to pass. Lots of soaking may help.
The info you got is wrong. They can have lettuce as a small part of a varied diet which you need to add more items to. They need proper heat, light, enclosure, substrate, humidity, diet, hydration, none of which it sounds like you are doing.
