The tortoise looks fine to me it doesn't seem like shell rot because the shell is firm. It could be because of low calcium intake, get reptile calcium powder (3$ for a lot) and shake it onto your torts food. This will make the sell grow over the white lines..
I would recommend a mercury vapor bulb it provides heat and uvb all in one. Problem is that they will work for 15 months or so and then stop providing uvb and only make heat. I fixed this problem BBY marking the day I got the bulb and then change it in exactly 15 months.
Get a porcelain clamp lamp. The Important thing is that you keep in mind the fact that the lamp is like a flood it doesn't direct the light in one direction the cage will be warm if your tortoise is within one foot of the fixture so this works better for cages at least 2x2

Many tortoises are slowing down mine slept the whole day yesterday. Everything should be fine but you are keeping him a bit dry add some humidity to his cage. Also about the hibernation give your tortoise a few soaks he should become more active.
