Alireza, Jareds head looks quite dry. And from the angle of your pic it looks like his nares may be clogged? Looks like Jared needs more hydration IMHO.
Do you give him a weekly 15-minute "bath"? Soak him in some warm water for 10-15 minutes shallow enough for him to keep his entire head above the water about once a week. This gives him a chance to drink and often times he'll do his business in there too.
What does your enclosure look like? Do you have a water dish and at least a small part of the enclosure you can keep humid?

when I put him in water when his head was under the water,he tried to clean his eyes with his hands.is it normal?does the water seepage to his nostrils?
Alireza, When you place your tort in water it should be to the bridge. That is where the bottom shell meets the top shell, this gives them plenty of room to keep their head above water because if they can't they can/will drown. When we say soak that means we place our torts in the water to the bridge for about 15-20 mins. Keeping the water tepid (warmed) in temperature so they don't get cold. It is also called a forced soaking or a bath. If he put his head underwater and wiped his eyes it is more than likely because he is dry or dehydrating. If he put his nose underwater that is how they drink.
What type of substrate do you have him on? Do you know what the humidity is in his habitat? Do you have a source of water he can get in and out of by himself in his habitat?
If a tort gets dehydrated it can create problems like kidney or blader stones, just to name a few. So you want to make sure he is well hydrated.
