not really, they can be the same, but MBD is usually the flat look the mushroom cap, or the one side slope look of the shell. It also causes weak bones which can affect walking. Pyramiding is the bumpy look to the shell, the causes for them are usually different.

Doesn't MBD usually show up in x-rays, in the leg bones?

I find confusion arises over the fact that MBD and pyramiding are often seen together. Usually, folks who are so ignorant of a tortoise's care as to allow the incidence of MBD are usually prone to offer a nutritionally deficient diet alongside a poorly maintained environment, which, of course, results in pyramiding. However, many times, some keepers who are reliant upon the widely available bad information have animals that suffer purely from pyramiding (as a result of environmental mismanagement) but are otherwise healthy. Humidity, regular access to water, and soaks are among those things that all tortoises require; they simply have different ways of acquiring these conditions depending upon habitat type. Regardless, no habitat on earth that supports tortoises bears resemblance to the sterile, cool and dry environment that is characteristic of most homes, the sort of environment that is certain to cause rapid dehydration and pyramiding.
