You can't rely tell except for their behavior. Loss of appetite, excessive sleep, sluggishness, runny poop, gook in the eyes, expelling bubbles from the eyes nose or mouth, open mouth breathing, etc, all of these are signs of illness. A natural de-wormer is pumpkin puree like the kind from stores.

Tortoises take time to adjust to their habitats. Has she eaten, passed any feces, peed, urates, drinking water etc. How old is your tortoise? Did you get an adult or a baby?

A good idea is to have a plywood enclosure that is massive, and then have one long end in the front be plexiglass. Halfway down the plaxiglass, you can screw in a very thin piece of wood so the tortoise can't see out, but you can see in.
