Aquatic turtle skin ripping

Yesterday I saw a hole in my Florida red belly cooter's skin, between her back leg and tail. Near that I know she had a scrape from my other one's nails by accident, but it had been healing for a while. I thought it had reopened so I went to clean it and saw a clean hole in her skin that was bleeding if you touched it. It does not look like a bite mark and it has gotten bigger since then. I cleaned the pool again today and have them separated just in case. I have a vet appointment today. She is acting fine and eating and swimming like normal, but I'm hoping she doesn't move around too much and make it worse. If anyone has any idea how it may have happened it would be super helpful and appreciated as I have never heard that happen to anyone

If it keeps bleeding when she moves, put her on dry dock in a small container and cover it making it dark and hopefully she will settle down and not move, at least until your vet appointment.
If you post pics of it, someone might be able to give advice on what to do or what it looks like that might have done it.

Thank you! I brought her in. She is a very calm turtle but I will keep checking and making sure she doesn't move too much. Thought maybe the water would soften it, plus she needed out. I will try and get a photo if I can. Thank you again

Update: Her diet needs to be changed, the mostly greens is not working for her. If anyone knows what deficiency that means that would be great. She got a light sedation, the hole was closed (kept closing it and it kept ripping her skin but eventually it worked), got a cream that needs to be applied every twelve hours, and an injection every three days in her arm.
Fairly recently I started giving more greens since I didn't like the pellets I was giving and she stopped eating the pellets. We don't feed any kibble to our dogs so I stopped with the pellets, but since she was used to them, now there's a deficiency. I don't want that to happen to my others so I will be attempting to find a good brand. I would very much appreciate any recommendations. Since this happened so quickly I am expecting her to have a fairly short recovery time. I will be keeping her mostly dry, only very low water and watering her carapace, along with separating them so she doesn't get scratched again.
Again, any food recommendations for her would be greatly appreciated, including supplements and vitamins.
