RI question?

Friend says her tortoise is making "clicking" noises. I told her to keep an eye on it, as it seems to show no other signs of anything. But I noticed a few days ago she posted this photo, and I'm wondering if he's just really enjoying the carrot..or if he's sick..

Looks like the juicy is coming from the eating....
