Is your tortoise kept indoors or out? Your tortoise needs to be kept warm and to be taken to a good vet that specialises in tortoises. It will need antibiotics. You should soak your tort in warm water for at least 30 minutes every day to keep it hydrated. If you post pictures of the enclosure and lighting we can recommend changes that will help your tortoise.
