Could this cypress mulch incident have injured my tortoise?

I was feeding my little yearling Hermann's yesterday and he came running like he always does. His substrate is coconut coir covered with cypress mulch. There was a really long piece of mulch with one end buried under a rock. Carl got stuck on the other side of mulch that was pushing into his little neck flap. He kept trying to walk forward but he was stuck on the piece of mulch.

At the time I couldn't stop laughing and eventually had to help him get out of the predicament but now I look back on it and wonder if Carl could have been injured.

If he's clearly too stupid to work around the mulch, would it be safer for him for me to just not use mulch at all?

This is one of the reasons I dislike cypress mulch.
