I had my tortoise in his outdoor enclosure, when I went to check on him I noticed that the two main scaled/skin above his nose had pulled off somehow. Is this anything I should worry about?

I don't think so, I don't think you even have to worry about infection since it does not look like the skin was broken.

Ouch! I wonder where he was poking his nose that it didn't belong. It should be ok. If you're worried about it, you can dab a small bit of NeoSporin on it, but don't clog up his nostrils.

It does not seem like it pains him at all so I will leave it alone. Thanks for the reassurance!

I wonder how it happened.. hopefully it'll heal up and grow new scales.

Almost looks like he poked his head through a gap or hole in the border, and when he retracted his head, his two nasal scales just got stripped right off. Might wanna inspect his outdoor enclosure so that doesn't happen again.
