My tortoise has weird spots

My sweet girl wasn't always mine and before I started caring for her she wasn't kept right for the first few years of her life which lead to a bunch of issues down the line we been in and out of the vets Office so many times trying to figure out how to treat what and how to keep her best. Recently she was refusing to eat properly and she doesn't rlly drink much at all I have to soak her and her food to get the fluids in she's not as active lately either and yesterday I noticed these dark little patches on her that weren't there before which rlly worries me.(also excuse my birds screaming I brought them into the room since I'm cleaning their enclosure)

You may have covered all this before, but if we can get a refresh on overall current care, it’ll be super helpful for members coming along to advise

What are your temperatures like all over? Ie basking temp(directly under the bulb), overall day temps(middle and cooler side), night temps?
What kind of basking bulb is being used specifically? Packaging photos are good if you have any
Do you use indoor uv? If so what kind?
How’s humidity?
What’s his substrate?
What kind of monitoring do you have?
What’s his diet been like with you?
