My Indian star tortoise, 4 yr old, female is urinating yellow and not eating since a few days. She has also been mating for a month now. Is she pregnant? Does she have any health concerns?

How is your tortoise kept? What does her enclosure look like? What is she fed?
It sounds like she has company. A 4 year old Indian Star doesn't sound big enough to breed. If she is being constantly mounted by another tortoise she is being harrassed and this is VERY stressful.
Tortoises are not social creatures. They don't get lonely and don't need or want friends. Another tortoise is simply a rival for food and territory. Mounting is used as intimidation by a dominant tortoise as well as for breeding.
This tortoise needs to be kept entirely separately in its own properly set up nclosure. Unless you do this your tort will decline further.
