Scale Problems

Hi all, having a bit of trouble with Vera :/ a week ago I went to check on her and noticed a scale on her head was missing and there was a little blood, I changed her onto repticarpet to prevent infection and cleaned with water, it was healing nicely until yesterday when the same happened to the next scale along, I noticed she was climbing and falling off of a log roll I got her recently so have removed it just in case,
Is this likely physical damage or could it possibly be a disease
I've also noticed for a while a part of her shell just above her head sticks out a little so I'm thinking after she caught her scale on something maybe the next one got caught on her shell. I've attached some images

Neither can I, unfortunately.
If there's a a sharp bit of carapace protruding it would be fine to gently file it smooth.
Sounds like physical damage to me, rather than disease.
