Noticed today and cleaned it with water. He is very adamant about moving his hide home right now and in worried that's how he got this.
He was adopted from a place that didn't really care for him so he has gotten a lot stronger in the last year than he was but I'm worried he pushed it too far.
I try to soak him about every 2 weeks. Now I am concerned about what to do / how much I should be worrying about this. Any help is appreciated!

Keep it clean and put some antibiotic cream on it. Do this daily.
His beak is over grown and needs trimming.
Also, attach the hide so he can't move it.
How big is his enclosure? As an adult, he needs a minimum of a 4x8 foot.
He needs a incandescent flood bulb for basking of 95-100 and an enclosure that's ranges from 75 to 80 all other areas. A tube florescent for uvb. The temp can drop into the 60's at night. Humidity should be 30-50%. With a big enough enclosure this will be fairly easy to accomplish and with some plants maybe he will leave the hide alone.

Just looking at his face he has an overhanging beak. I recommend you contact a local vet or reptile shop to help you trip and tidy up beak and claws etc.. now your the new owner please read the care sheets and adjust settings as required.
Introduce regular shallow warm water soaks (3 a week) for 30 mins in a high sided container. This helps hydration, excrete and exercise etc.. for the crack I think the new growth due to good diet and calcium will help that heal. I would put some antiseptic cream along the scute to avoid any fly's laying eggs.
