My tortise's shell looks very unhealthy what can I do to fix this issue.

To me it just looks really, really dry. Try rubbing in some cold pressed coconut oil, paying particular attention to the 'seams', allow it to absorb for a few minutes, then polish it off with a clean absorbent cloth.
