I feed her a mixture of green leaf lettuce, orchard hay and tortoise diet pellets. The temp stays around 90-95 during the day with humidity of 40-55 and at night the temp is around 80-90 with a humidity of 50-60. I soak her on Monday and fridays and he substrate is coco fiber from eco earth. She has basking light and 10.0 uvb light and night light as well.

Humidity needs to go up, 80 always.
Basking temp should be 95-100 but the rest of the enclosure should be 80-85 day and night.
If the enclosure is big enough, not a small aquarium then the temp gradient should be easy to reach.
Soak should be daily for about two years then can go down to a couple times a week.
She is already pyramiding. Get changed done ASAP and do not use a mercury vapor bulb if you are and should be a closed chamber enclosure.
Needs more items to diet too.

Great advice! Please try to follow it for your beautiful little tortoise's sake. And please, no night light. She needs it warm, humid and DARK at night, like in her natural native habitat.
