URGENT!!!! (soft shell)

My 4 year old hermanns tortoise has been pyramiding since we got her (when she was 3) and i have tried everything to stop it and now parts of her shell are soft. What should i do? Please help!

What are your enclosure temps? Night heat? Basking area? Cool side? Hot side?
What's the humidity?
What type of uvb/uva bulb do you use?
What do you feed it?
Please post pictures of the tortoise and enclosure.

How old are the UVB bulbs you are using? Does she ever get natural sunlight? Are you giving any calcium supplements?
All of which needs to be done if your not. UVB bulbs are only good for 6 or 12 months depending on type of bulb and may actually be shorter without a UVB meter there's no telling.
