The turtle is an ornate box turtle, Terrapene ornata, with a grossly duck-billed overgrowth of the beak or mandible and overgrown nails.
Those issues will need attention. The turtle's condition suggests it has been neglected and has not received appropriate care and culture.
Therefore it probably has a myriad of other problems to address.

I know nothing about box turtles, but here is a link to the box turtle care sheet in the box turtle species specific forum. I hope you can get this turtle the care it needs. Box Turtle Care Sheet

Hi Jay, I see this your first post on TFO, welcome!

The observations that the bill/beak and nails are overgrown is solid, that there are myriad of other issues may or may not be true, best to deal with what you can that is apparent. I agree it is an Ornate Box turtle, the ID is solid.

Best diet for adult ornate box turtles is insect heavy omnivore with some produce.

The extent of the two issues which are pretty advanced, beak and nails, would best be dealt with by a veterinarian. There is a vet directory here on TFO access from the main menu. If you have alot of animal handling experience and especially turtle experience you might try dealing with it yourself. Otherwise the vet is the way to go.

Many box turtle keepers will follow-up with you if you go to that part of TFO, and link back to this post or start from the beginning with your question.

The store owners either sold their last copy of Tess Cook's "Box Turtles" about 10 years ago or they don't practice what they sell. Hello Jay, Welcome to the Forum. Hopefully your efforts to be on here can relay to the owners to do the same. This Male Ornate Box Turtle will continue to deform under the current conditions. I hope your interest can help this guy.

Tess's book is in print and there is a free online version as well. I recommend both highly.
