That form say it expires in 2020. I thought the permit was valid for 5 years? Also, does it matter what state the form says it from?

This current version of the form expires in 2020, so there will be an updated form made available at that time. This is the form to use until then.

Permits are valid for 5 years from the date they are issued.

This is the form used for each State. The federal office that processes the form applications is located in Virginia.

There is also a $200 "nonrefundable application processing fee" that must be sent in with the application, in the form of a check or money order.
