Identity crisis!

A friend of mine recently acquired a turtle/tortoise from someone who really didn't take care it. We need help identifying the species, what diet it should have, any lighting or other habitat info would be very helpful as well. Thanks for the help!

Its a kind of box turtle (terrapene) I'm really really bad in distinguish the different kinds of this genus. Wait for Yvonne she is able to identify this guy 100%

Looks like a very young, poorly cared for three-toe box turtle (Terrapene carolina triunguis). But it might be a different subspecies. Sometimes they're hard to tell until they start to get their grown up look to them. Here's a generic box turtle care sheet:

Thanks for the quick responses everyone! We live in south-central Pennsylvania. She also made a vet appointment to make sure there are no worms or other issues, along with having nails trimmed. The person who found this turtle, and took it from the wild (it was a few years ago) really neglected it, but it's in much better care now.
