Buy some Miner-all, mfg'd by Sticky Tongue Farms. It provides trace minerals your tortoise may be looking for in his substrate.

There is dirt unnder the bark that was not there before the tortoise is a hermann's tortoise about 2 years old and is 4 inches. I feed her calcium every friday.

What lights are you using?
Any colored bulbs?
What is the diet?

I am using a UUVB bulb and a heat bulb I feed her mostly weeds. When I do not feed her weeds I feed her lettuce,kale,collards,carrots,watermelon, and cucumbers.

Is the heat bulb a colored bulb?
Is this your HERMAN'S tortoise?
Watermelon should be avoided and carrot needs to be a very rare treat.
Also, what type of UVB "bulb"?

The Exoterra is most likely a MVB.
Not a good choice for UVB. And they can get very hot.
They will sometimes make tortoises uncomfortable. But that wouldn't explain eating the substrate

26w UVB 150 from exoterra this is a temporary enclosure
That is a cfl type bulb. It is likely burning your tortoises eyes and that might be why it is eating the substrate. I would turn that off and get rid of it ASAP.

Use Arcadia or ZooMed HO tubes for indoor UV when needed.
