How close should the lights be to the enclosure

Most of them have directions stating no closer than 12" (from the light to the tortoise's back) If you're talking about redfooted or yellowfooted tortoises, then you need to diffuse the lighting. Lots of plants help in that regard.

Remember that Redfoot don't like it very hot and get stressed out by the light being too bright.

You may want a small CHE for heat and a strip florescent tube to handle the UVB.

Have uvb bulb & a 40w heat bulb uvb 12 on 12 off
