My guess is that he was raised too dry. Here's a good explanation about baby tortoises: https://tortoiseforum.org/threads/what-you-need-to-know-about-baby-tortoises.178141/
Could it be from before we got him as a hatchling? Since we've had him he soaks himself several times a day, sometimes he sits in his water for hours. I soak his pellets to expand them and to make sure he is ingesting water, also since we have made his new enclosure we spray down it down every day.

Show us pictures of his entire enclosure and note your heating and lighting details.. also tell us about his diet. Maybe we can give you more exact information. Have you read and followed all of the care sheets here? Do you know how old he was when you got him and how he was kept prior to? I know it’s a lot of question, but all relevant if your looking for exact answers..

I have learned a lot of things in the 4 hours I have been on this site this morning!!!!! Thank goodness I found this. I am going to have to make him a new enclosure for sure now!

Is is in a 48in x 48in table box. He has been in this new box for about a month. He was in a 20 gal tank before (terrible I know, I had no idea). His first enclosure was kept at 80 degrees and the light was directly over his water and he lived in that water dish 50watt image.jpg bulb. His substrate was reptibark and he had a wood hide.
Okay, back to the new table box. 2ftx2ft, substrate is organic potting soil and Cypress mulch. I have flat basking and feeding stones, and rocks that he likes to move around. We just purchased a 100wat lamp last night because it's getting colder outside and in the house. The house is at 71. I'm so paranoid now about his heat that I hooked up both lamps. His hide is cool and damp (I'm thinking the coolness is bad). The soil in there stays damp and makes it feel cold. I spray down the enclosure once a day and I soak him everyday I don't see him do it himself.
I do not know how oldbhe was when we got him, just that he was hatched recent to getting him, maybe a month or two old? He was the size of a silver dollar or a little bigger. I have had him since Jan of 2019.

Well it sounds like you found all your answers and your making adjustments! We are here to help, not criticize.. do you still have any questions? I think you’ll start seeing a big difference when you get his heat and humidity regulated. He’s still a beautiful little tort!

Also, there are plenty of ways to rig what you’ve got into and enclosed habitat temporarily .. that will help a lot

He’s going to need a lot more space though!
