Hi,
Can you tell us more about your tort - species,, age how long you've had him?
Are you sure you have a male?
Does it retract back into the cloaca?
I'm wondering if it's a prolapse or a penis - I don't know but I'll tag some people who will @Yvonne G @Tom @zovick @ZenHerper and I see @Toddrickfl1 has signed in so hopefully one of them can help.

Until someone with more knowledge about this comes to help, maybe try a luke warm soak to stop it drying out then don't put him back on his normal substrate but keep it clean.

It looks like the tip of his penis, is it coming out of it's closca, or attacked to the outer shin. Tail looks a little short for a male. Just my opinion

This looks like a female, in which case it's her female parts being expressed. This could be caused by being egg bound, constipated, or straining extra hard to get something out (hard urates, stones, etc.) Does the red blob stay out or does it go back inside? Dip your fingertips in vaseline and , with the pads of your fingers, gently push the blob back inside. If you're able to get it back inside you may need to just sit there a while holding it in until she stops pushing. It's imperative that you figure out what's causing her to strain so hard because unless that's fixed it will keep happening.

That is not a cyst. It is a prolapsed cloaca, possibly from straining to pass a bladder stone or an egg. The tissue is beginning to dry out, which will cause further trauma.

You may be able to get the tissue to reduce/retract by using a wet sugar compress and then trying to replace the prolapsed tissue into the cloacal opening in the tail, but that doesn't address the cause of the problem.

If you have a good reptile vet near you, then I recommend having the tortoise checked by the vet rather than trying to treat it yourself at this stage of the game as the tissue looks quite irritated already.

^^^ that. manuka honey may also work but i agree they should be seen by a vet as soon as possible. if that tissue is out to long it will die and then surgery will prob be needed or worse.
