This animal needs to be seen by a tortoise vet. You'll have to find one, near to you or not.

Sadly, your tortoise is showing at least two of the signs of Austwickia chelonae. Yellow bumps on skin, loss of toenails and horny portions of the beak are all tell-tale signs. This is an infection that will ultimately cause the tortoise to die whether it is treated or not. The most humane thing to do is to euthanize the tortoise.

Let's get another opinion, though. @mastershake Do you agree that this tortoise has Austwickia?

sadly yes this looks to be a case in pic two look at the inside of the arm, as well as the other signs are tell tale in almost every single case (we have seen nearly close to 200 now). i feel terrible having to say this. if this is qustwickia there is nothing that can be done for them. if you decide to test they need to do a very deep resection of skin prob on the inner arm there it needs to be all the way through and will require a stitch or two. otherwise if they do pass they can be sent out to one of a few labs we now work with in testing for this. we have a few now around the country who can necropsy for this specifically. if you have any questions you are welcome to ask. by any chance are there any toenails on the back feet missing? this would be a yet another symptom besides the others
.
