Substrate + tortoise health

Hi all, please can someone recommend the best substrate for a 4 year old marginated tortoise.
And,
Ive posted recently about health concerns and it seemingly hasnt got better can anyone determine what is wrong with it , charges around, seems to struggle breathing , but then stops moving completely and sits under the lamp and wont eat, while the bottom of its shell is soft and seems to be gaps between. Any help would be appreciated as becoming a concern

The best substrate for an indoor enclosure is fine grade orchid bark. Outdoors the native dirt is best.

Visually, I don't see a problem on the plastron. If it is soft, then there is a problem. We need to know how you are housing the tortoise. Pics of the enclosure would help.
What are your four temperatures?
What substrate?
What heating and lighting?
What type of UV bulb?
What are you feeding the tortoise?
Any supplements like calcium?
How often do you soak the tortoise?
