Sick tortoise (red marks on lower shell, no eating, urination or defecation)

I have an ill tortoise, its lower shell turned a bit red about 3 weeks ago, no eating, urination or defecation, what should I do? We gave it marbocyle 2 times, according to vet, but he turned out to be not specialised in reptiles, then changed to ceftizidime, here's a photo before it got sick, the other one is of the shell

That tortoise looks very sick with possible septicemia, you will need to see a vet asap. Systemic antibiotics, fluid therapy, and nutritional support are needed. I would seek out an exotic vet asap.

Is the tortoise still on the Ceftazidin antibiotics?
How is he doing today?
Are you keeping him isolated?
Do you have other tortoises?

How is he now? I really don't think treating him without a vet is possible. I am tagging @zovick again.
I don't think simply treating the tortoise with antibiotics will cure this problem completely. You should try to find a good vet.

Is that an Egyptian Tortoise?

Also it looks as though some of the scales on both of the front legs are missing. Is that correct?
